Referral Information for Physicians

Note that referrals can only be initiated by your physician. If you are a patient who wants to be seen at the Sleep Disorders Clinic, you need to talk with your doctor first who can then refer you to us.

Please download the following referral form and FAX to 604.822.9744. This form will be reviewed and depending on the clinical problem, an appointment made with either a Respirologist or Psychiatrist affiliated with the program. If you need to speak with one of the staff here directly, please call 604.822.7606.

Waiting Times

Maximum approximate time from physician referral to complete assessment at the Sleep Disorders Program:

• Priority 1 (urgent): 4 weeks
• Priority 2 (semi-urgent): 8 weeks
• Priority 3 (elective): 15 weeks
